North County Pop Warner Acknowledgement Form
Due to the fact that we are part of a National Organization we must adhere to strict regulations within
our own association. In order for the program to run successfully we require parents to cooperate
with our policies and procedures. Please indicate your acknowledgment of the following policies by
signing below.
Paperwork Policy: All paperwork is due by April 1, 2012 for the Flag Season and by July 1, 2012 for the Tackle
and/or Cheer Season. If all paperwork has not been submitted, your child may not be issued equipment or
may not be allowed to attend practices. If failure to submit Tackle and/or Cheer paperwork continues after
August 1, 2012, your child may not be allowed to participate in NCPW in any capacity during the 2012 Season.
Paperwork may be mailed to NCPW, PO Box 552, Brush Prairie, WA 98606.
Payment Policy: All registration fees must be paid by April 1, 2012 for the Flag Season and by July 1, 2012 for
the Tackle and/or Cheer Season.
Refund Policy: As a general policy, registration fees are non-refundable. This means that once a
parent/guardian registers a child in the program and pays the fee, the fee will not be returned if a
parent/guardian makes the decision that their child will not participate for any reason. Registration fees are
refundable if a child is registered in the program but NCPW is unable to roster the child on a team. These
circumstances can include but are not limited to the following: a child is waitlisted on a team due to the max
number of players allowed per team, the child cannot be placed on a team due to age/weight restrictions or
there is not a team in the division needed or if there is a change in your child’s medical history accompanied
by a Doctors excuse. If any above circumstance arises a refund request must be submitted in writing.
Parent Policy: NCPW is run 100% by volunteers. Due to the fact that there are many more jobs to be done
than hands available to do them, we require the participation of the parents during the season. Your team
mom or coach will be asking parents to assist with some of the tasks required to run the program. Please be
available as much as possible to help our season run smoothly.
Fundraising Policy: We are a non-profit organization and our registration fees alone cannot provide a budget
needed for a successful season. Our revenues are devoted to the safety, support and recognition of the
children who participate. Practice fields, equipment, uniforms, lights, safety equipment, playing fields for game
days, storage facilities and insurance are just some of the many expenses our organization incurs. We have a
need each year to raise additional funds or accept sponsor donations along with player fees to cover these
expenses and hopefully grow as an association.
As indicated by my signature below, I agree to adhere to each of the stated policies of North
